Yep. Time to go old-school and use actual pen and paper. But there actually is a medical/release form that must be filled out and submitted by all participants. Also, a background check form that must be filled out by all adults.

Step Three:

Submit Deposit



As you might imagine, the growth of — and interest in — this trip makes the planning more complex, and so the sooner we know who is going, the better. As we did last year, we are collecting a deposit of $250 from each participant, in order to secure his/her place on the trip.



Submit your deposit via credit card here. (You will be asked to enter an amount first — please enter $250 for each registration. Be sure that "Kentucky MIssion Trip" is selected in the pulldown menu and complete the required steps from there. You may then close that window, when finished, to return here).



If you would prefer to send a check in directly to New Life, please do two things:


  1. Make the check out to New Life Christian Church.

  2. In the memo line, write: "Kentucky Mission Trip Deposit for <insert your name or name of person  your payment will support>";

  3. Mail to:
